The FAA grounded all MD-11 and MD-11F aircraft for inspections and necessary corrective actions.
The National Transportation Safety Board (NTSB) released its preliminary report on the United Parcel Service (UPS) cargo flight that crashed after taking off from the Louisville Muhammad Ali International Airport (SDF) in Kentucky. The crash killed the 3 crew members and 11 people on the ground, while injuring 23 others.
The NTSB found that fatigue cracks in the hardware of the engine mount caused the engine to separate from the airplane wing as the plane lifted from the runway. The plane was able to climb just 30 feet in the air before it crashed. The jet had a full tank of fuel, which resulted in a half-mile burn field of debris.
NTSB will be reviewing all the maintenance records of the aircraft before the final report is issued in 18-24 months. The Federal Aviation Administration (FAA), UPS, The Boeing Company, Independent Pilots Association (IPA), General Electric (GE) Aerospace, and Teamsters Airline Division have participated in the investigation of the crash.
